Support Worker – Community Rehabilitation (Part-time)

About the role
We are seeking a Support Worker to assist a male client who has sustained a brain injury. The role is based in the client’s home in Hayes and involves two 12 pm‑8 pm shifts per week, providing person‑centred rehabilitation support.
Key responsibilities
- Support the client in achieving his rehabilitation goals.
- Provide personal care, medication administration and manual handling assistance.
- Liaise daily with health‑care professionals and update care plans.
- Maintain a safe and supportive home environment.
- Attend support worker meetings and person‑centred training sessions.
Required profile
- Minimum 1 year experience in health or social care.
- UK driving licence.
- Psychology graduate with rehab experience.
- Basic understanding of developmental delay and cognitive impairment.
- Excellent verbal and written communication.
- Good IT proficiency.
- Willingness to learn, accept feedback and think creatively.
Required skills
- Medication administration.
- Personal care provision.
- Manual handling techniques.
