Support Worker – Homeless Services

About the role

This temporary, full‑time position supports individuals at risk of homelessness or currently on a homeless pathway. You will help improve their wellbeing, connect them with local community resources, and assist with managing their homes.

Key responsibilities

• Manage a personal caseload of clients navigating homelessness.
• Conduct initial and ongoing assessments of needs and risks, agreeing on support levels.
• Create and maintain individual support and action plans.
• Provide practical assistance with domestic tasks to promote high‑quality accommodation.
• Develop and sustain links with key agencies and service providers.
• Ensure clients receive entitled services and benefits.
• Maintain accurate tenant records and complete required administrative work.
• Encourage rent payment and coordinate with the Housing Management team.
• Proactively chase referrals to maximise housing placement efficiency.
• Carry out health and safety duties as required.

Required profile

• Experience supporting vulnerable adults, particularly those at risk of homelessness.
• Ability to assess needs, identify risks, and develop tailored support plans.
• Strong organisational and administrative skills.
• Excellent communication and teamwork abilities, with a collaborative approach to external agencies.
• Commitment to empowering clients to live independently.
