Support Worker – Youth Transition Service

About the role

As a Support Worker in the Transition Service, you will help young adults aged 16‑early 20s move from school into independent adult life. Working Monday‑Friday, you will accompany them in community activities, build confidence and support the development of daily routines.

Key responsibilities

• Support small groups of young adults in community settings such as cafés, gyms, or recreational activities.
• Encourage individuals to try new experiences, develop confidence and independence.
• Assist with managing changes in routine and new situations.
• Provide one‑to‑one support when required, including calm handling of distressed or challenging behaviour.
• Build positive, person‑centred relationships and maintain a safe environment.
• Keep accurate records and contribute to support planning.

Required profile

• Reliable, patient and motivated to support young people during a key life stage.
• Willingness to learn and work both in teams and independently.
• Ability to use initiative in community settings.
• Commitment to person‑centred, respectful support.
• Willingness to join the PVG Scheme and register with the SSSC.
