Team Leader – Care Home

About the role

Avery Healthcare is seeking a compassionate Team Leader to join the award‑winning Aire View Care Home in Leeds. You will support the home management team in delivering high‑quality, person‑centred care while fostering a safe, inclusive environment for residents, visitors, and staff.

Key responsibilities

• Lead the person‑centred care planning process, including monthly evaluations, care plan audits and action‑plan implementation.
• Oversee smooth resident discharges, ensuring accurate documentation, medication management and communication with next of kin.
• Monitor resident well‑being, identify changes affecting care, and update records to reflect new needs.
• Promote safe working practices, conduct risk assessments and ensure safe moving and handling of residents.

Required profile

• NVQ Level 3 (or equivalent) in care for care homes.
• Minimum two years’ experience in a senior care role.
• Strong verbal and written communication skills.
• Ability to lead staff and act responsibly in the manager’s absence.
