Team Leader – Care Home

About the role

Avery Healthcare Group is seeking a compassionate and enthusiastic Team Leader for its award‑winning Edenbridge Manor Care Home. In this full‑time position you will help create a safe, caring and innovative environment for residents, visitors and staff, while supporting the home management team in delivering high‑quality care.

Key responsibilities

• Lead the person‑centred care planning process, including monthly evaluations, care‑plan audits and action‑plan development.
• Facilitate smooth resident discharges, handling documentation, medication and communication with next of kin.
• Monitor resident well‑being, identify changes affecting care, and update records accordingly.
• Ensure safe working practices, assess risks and promote safe moving and handling of residents.
• Act as a leader in the manager’s absence, providing guidance and support to staff.

Required profile

• NVQ Level 3 (or equivalent) in care for care homes.
• Minimum two years’ experience in a senior role within a residential care setting.
• Strong verbal and written communication skills.
• Demonstrated ability to lead staff and take responsibility when the manager is unavailable.

Required skills

• Knowledge of care‑plan development and audit processes.
• Risk assessment and safe moving‑and‑handling techniques.
• Effective documentation and record‑keeping.
