Team Leader / Rehabilitation Support Worker – Complex Care

About the role

We are seeking a dedicated Team Leader / Rehabilitation Support Worker to provide person‑centred care for an 18‑year‑old male client with complex neuro‑rehabilitation and spinal injury needs in Whyteleafe. You will work within a multidisciplinary team to support the client’s transition from hospital to home and promote independence.

Key responsibilities

• Deliver high‑quality, person‑centred care in the client’s home.
• Support rehabilitation programmes and therapy goals.
• Assist with manual handling and mobility support.
• Administer and monitor medication safely.
• Collaborate with healthcare professionals and therapists.
• Monitor progress and report changes in condition.
• Provide emotional and psychological support.
• Help prevent complications and promote long‑term wellbeing.

Required profile

• Experience supporting individuals with spinal cord injuries and complex needs.
• Strong manual handling skills.
• Experience in medication administration and monitoring (hospital/ICU background desirable).
• Understanding of rehabilitation and recovery pathways.
• Excellent communication and interpersonal skills.
• Good IT and documentation abilities.
• Ability to work independently in a client’s home.
• Willingness to learn, adapt, and take feedback.
