Transition Lead – Supported Internship Programme

About the role

We are seeking a passionate Transition Lead to support young adults with SEND as they move into, through and beyond the Buckinghamshire Supported Internship Programme. Based in Aylesbury, you will work closely with interns, schools, employers and the local authority to ensure smooth transition planning and promote independence and long‑term employability.

Key responsibilities

• Lead transition planning for interns, preparing them for entry into the Supported Internship Programme.
• Provide coordinated, person‑centred support to help interns progress into paid employment, apprenticeships or volunteering.
• Collaborate with schools, employers, the local authority and the Supported Internship team to deliver tailored support.
• Monitor and reduce anxiety, build confidence and develop independence among interns.
• Maintain accurate records and produce reports on transition outcomes.

Required profile

• Passionate and motivated with a genuine interest in supporting young adults with SEND.
• Excellent communication and interpersonal skills for multi‑agency collaboration.
• Ability to work independently and as part of a team in a fast‑moving environment.
• Commitment to person‑centred approaches and promoting long‑term employability.
