Ward Liaison Officer

About the role

The Ward Liaison Officer supports safe, timely patient discharge by coordinating with ward staff and discharge teams. Working on the front line of the hospital, the role ensures that beds are available when needed and that patient flow aligns with Trust policies.

Key responsibilities

• Assess discharge options for each patient and act to resolve any delays.
• Communicate daily with discharge nurses, ward staff and multidisciplinary teams to align on discharge plans.
• Visit allocated wards regularly to monitor patient and care flow, updating records as required.
• Collaborate with external contacts and stakeholders to facilitate seamless transitions from hospital to community care.
• Adhere to Trust guidelines, departmental protocols and NHS standards throughout the discharge process.

Required profile

• Experience working in an acute or community healthcare setting.
• Strong communication and persuasive skills to influence discharge decisions.
• Ability to build effective relationships with clinical and non‑clinical staff.
• Understanding of NHS discharge pathways and patient flow management.
