In this pivotal role, you will act as a clinical expert, driving excellence in patient care, optimizing therapeutic outcomes, and leading specialized interventions within a multidisciplinary environment.

Missions principales

• Provide advanced clinical assessment, treatment, and continuous follow-up for patients with complex heart failure portfolios.
• Optimize pharmacological and non-pharmacological evidence-based treatments to improve patient quality of life.
• Lead patient education programs for individuals and caregivers to promote self-management and reduce hospital readmissions.
• Facilitate safe and efficient discharge planning and manage clinical telephone triage and referrals.
• Contribute to service development through clinical consultancy, teaching, audits, and nursing research.
• Act as a key liaison between hospital clinicians, General Practitioners (GPs), and multidisciplinary teams.

Profil recherché

• Proven expertise as an Advanced Clinical Nurse Practitioner in a specialized cardiac setting.
• Demonstrated ability to manage complex caseloads and make autonomous clinical decisions.
• Strong leadership skills with a commitment to clinical governance and professional standards.
• Ability to work effectively within a multidisciplinary team to ensure patient-centered care.
• Non-medical prescribing certification (or currently working towards it) is essential.
