Hospital Adult Social Worker – London

About the role

We are seeking an experienced Adult Social Worker to join a hospital‑based team in London on a full‑time basis. The role focuses on managing complex hospital discharge cases, ensuring safe and timely transitions from hospital to community care.

Key responsibilities

• Undertake complex Care Act assessments, reviews and support planning.
• Manage hospital discharge cases, coordinating with NHS colleagues, OTs, health professionals and families.
• Assess care and support needs and arrange appropriate packages of care, re‑ablement and community support.
• Handle safeguarding concerns and high‑risk cases, applying professional judgement.
• Maintain accurate case records in line with statutory requirements.
• Provide guidance to colleagues and contribute to multidisciplinary meetings.

Required profile

• Social Work qualification with current registration with Social Work England.
• Significant experience in adult social care and proven hospital discharge experience.
• Ability to operate at Senior Practitioner level.
• Strong knowledge of the Care Act 2014 and adult social care legislation.
• Experience managing complex cases, safeguarding and risk.
