Occupational Therapist – AHP Long Term Conditions Service

About the role

The AHP Long‑Term Conditions Service at NHS Greater Glasgow and Clyde delivers a digital‑first, six‑week self‑management programme for patients with chronic health conditions such as ME/CFS, fibromyalgia, post‑viral fatigue and Long Covid. The role is hybrid, combining remote work, community home visits and attendance at local health centres.

Key responsibilities

• Provide specialist Occupational Therapy interventions to a defined caseload within the Long‑Term Conditions Service.
• Manage the daily functions of the Occupational Therapy service, ensuring quality outcomes for patients.
• Educate and develop other Occupational Therapy staff and multidisciplinary team members.
• Contribute to research and development activities related to long‑term condition management.
• Offer specialist advice and resources to patients, carers and external agencies, including sign‑posting to community and digital solutions.

Required profile

• Occupational Therapy degree (or equivalent) and HCPC registration.
• Demonstrable community experience with a range of long‑term health conditions.
• Valid driving licence and access to a car for service coverage across the health board.
• Ability to work autonomously as part of a board‑wide team.
