Pharmacist – Specialist Homecare Pharmacy

About the role

Baxter is seeking a qualified Pharmacist to join its specialist home‑care pharmacy team in Cheadle. You will be responsible for safe prescription management and dispensing of a range of home‑care therapies, ensuring patient safety and adherence.

Key responsibilities

• Act as the Responsible Pharmacist, delivering prescription management and dispensing across all therapies in line with KPIs and standards.
• Process prescription receipts, conduct clinical screening/intervention, and accurately dispense medications.
• Report near‑misses and dispensing errors promptly.
• Place product orders, authorise entries on the Patient Medication Record, and formulate unlicensed medicines as required.
• Collaborate with clinical teams to monitor patient adherence and escalate issues when necessary.
• Record and escalate adverse events to the appropriate escalation points.
• Provide clinical counselling and product expertise to patients, care teams, and pharmacy support staff.
• Mentor and coach pharmacy support staff, delivering associated training.

Required profile

• Registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Post‑registration experience in a home‑care pharmacy setting (advantageous).
• Experience conducting clinical investigations such as near‑miss investigations.
• Strong coaching and training abilities with a capacity to motivate and develop others.
• Critical thinker with solid problem‑solving and analytical skills.
• Commitment to continuous professional development and up‑to‑date knowledge of GPhC and RPS standards.
