Social Worker – Discharge to Access (D2A) Service

About the role

The Discharge to Access (D2A) team at Birmingham City Council provides short‑term support for individuals leaving hospital, helping them plan longer‑term care and, where needed, navigate complex housing and homelessness pathways.

Key responsibilities

• Conduct person‑centred assessments and develop care plans in line with the Care Act 2014.
• Provide guidance, sign‑posting and early‑intervention support using the 3 Conversations model.
• Manage cases within the Summer Hill Terrace Homeless Pathway, addressing housing, substance misuse and safeguarding issues.
• Collaborate with multidisciplinary teams to ensure coordinated service delivery.

Required profile

• Recognised professional Social Work qualification (MA/BA/B.Sc. in Social Work, Diploma in Social Work or CQSW).
• Registration with Social Work England.
• Minimum 2 years post‑qualifying permanent experience.
