Specialist Support Worker – Mental Health Social Care

About the role

We are seeking a passionate, compassionate and resilient Specialist Support Worker to join our Mental Health Social Care Service in Tower Hamlets. You will work with individuals who have significant offending histories and complex mental health needs, helping them achieve recovery and independence.

Key responsibilities

• Maintain a keyworking caseload of 4‑6 customers, delivering regular structured sessions focused on mental health, risk, daily living skills and recovery goals.
• Develop, review and co‑produce support, risk management and crisis plans that are outcome‑focused.
• Conduct initial and ongoing risk and need assessments, monitoring dynamic risk indicators and escalating concerns promptly.
• Provide practical day‑to‑day support, including budgeting, shopping, appointments, self‑care and routine building to promote independence.
