Healthcare Support Worker – Upper Annandale Community Nursing Team

About the role

We are seeking an enthusiastic Healthcare Support Worker to join the Upper Annandale Community Nursing Team. You will provide high‑quality nursing care to housebound patients, supporting a 24‑hour service across a diverse rural area.

Key responsibilities

• Deliver person‑centred care to patients in their homes, following clinical guidelines.
• Collaborate daily with a multidisciplinary Home Team, including allied health professionals, social workers and public health staff.
• Assist with patient assessments, medication administration, and health monitoring as directed by registered nurses.
• Maintain accurate patient records and communicate updates to the nursing team.
• Travel to patient locations safely, using a personal vehicle or arranged transport.

Required profile

• Passionate about community health and supporting vulnerable individuals.
• Ability to work independently in a changing environment.
• Current driving licence (or eligible for alternative transport arrangements if disabled).
• Flexibility to work part‑time, 25 hours per week.
• Preferred but not essential experience in a community health setting.
