Adult Social Care Practitioner – Locum (Full‑time/Part‑time)

About the role

We are seeking an experienced Adult Social Care Practitioner to join our locum team in Bromley. The role offers flexible full‑time or part‑time hours, with two days a week in the office, allowing you to support adults and their carers while maintaining a healthy work‑life balance.

Key responsibilities

• Promote independence and wellbeing for adults and their carers.
• Safeguard individuals at risk and respond promptly to safeguarding concerns.
• Assess needs in line with the Care Act 2014 and develop person‑centred care plans.
• Deliver and review support plans, ensuring high‑quality, effective support.
• Collaborate with service users, carers, partner agencies and multidisciplinary teams.
• Monitor and evaluate care delivery, contributing to service development and continuous improvement.

• Strong communication and teamwork skills.

Required skills

• Knowledge of the Care Act 2014.
