Adult Social Worker – Home from Hospital Team (Southampton)

About the role

The Home from Hospital Team at University Hospital Southampton is seeking an experienced Adult Social Worker to support patients leaving hospital safely and rebuilding independence. This senior position combines professional leadership, practice development and quality assurance within a busy multidisciplinary setting.

Key responsibilities

• Provide professional leadership, coaching and guidance to Social Workers and Social Care Practitioners.
• Support staff in delivering strengths‑based, person‑centred assessments and outcome‑focused support plans.
• Promote best practice in Care Act assessments, Mental Capacity Act decisions, safeguarding and risk management.
• Facilitate safe and timely hospital discharges in line with Home First principles.
• Deliver reflective supervision, mentoring and support for students and newly qualified social workers.
• Assist with complex casework, safeguarding enquiries and legal literacy.
• Conduct audits, quality assurance activities and practice improvement work.
• Collaborate with health, housing, community and voluntary sector partners.
• Deputise for the Team Manager as required and contribute to service development.

Required profile

• Qualified Adult Social Worker with senior‑level experience.
• Demonstrated ability to lead and develop multidisciplinary teams.
• Experience in hospital discharge and Home First services.
• Strong understanding of the Care Act, Mental Capacity Act and safeguarding legislation.
• Ability to manage complex cases and provide supervision.
