Advanced Clinical Pharmacist – Rheumatology

About the role

We are seeking a passionate Advanced Clinical Pharmacist to join our multidisciplinary Rheumatology team. The role will be based at an acute hospital site with flexible, semi‑remote options, and will involve full‑time work (part‑time and job‑share are welcome).

Key responsibilities

• Prescribe and manage home‑care prescriptions for rheumatology patients receiving biologic therapies.
• Coordinate with the home‑care department to ensure timely delivery and appropriate monitoring of biologic treatments.
• Handle drug shortages and biosimilar switches to optimise cost‑effective prescribing.
• Respond to home‑care and multidisciplinary team queries regarding rheumatology medication management.
• Develop, write and review rheumatology‑specific treatment guidelines.
• Manage patients on disease‑modifying anti‑rheumatic drugs (DMARDs) until stabilised and liaise with primary‑care providers for shared‑care prescribing.
• Participate in on‑call and weekend rotas as required.

Required profile

• Qualified pharmacist with a strong clinical background.
• Interest in rheumatology and willingness to develop expertise in the specialty.
• Excellent communication, interpersonal and time‑management abilities.
• Strong decision‑making and problem‑solving skills.

Required skills

• Prescribing and medication management for biologic therapies.
• Knowledge of drug shortage management and biosimilar substitution.
• Experience with developing clinical guidelines.
• Understanding of DMARDs and shared‑care prescribing pathways.
