Care Coordination Consultant

About the role

Join a well‑established live‑in care provider as a Care Coordination Consultant in Tonbridge. You will be the link between self‑employed carers and clients, ensuring that vulnerable people receive the right care at the right time while supporting carers throughout their placements.

Key responsibilities

• Build strong relationships with clients, families and carers.
• Match carers to clients based on skills, experience and personality.
• Manage schedules and maintain continuity of care.
• Provide ongoing support to carers during their assignments.
• Resolve challenges quickly and propose practical solutions.
• Maintain accurate records using CRM systems.
• Collaborate with internal teams to deliver a seamless service.
• Contribute ideas for service improvements and efficiency.
• Participate in the out‑of‑hours on‑call rota.
