Care Team Leader – Supported Living (Grimsby)

About the role

We are seeking a motivated Care Team Leader to support the Service Manager in delivering high‑quality, person‑centred support within our Established Supported Living service in Grimsby. You will lead a team of Support Workers, ensuring safe, independent living for adults with learning disabilities, autism, physical disabilities and mental health conditions.

Key responsibilities

• Lead, motivate and develop a team of Support Workers.
• Deliver high‑quality, person‑centred care to service users.
• Support recruitment, staff development and training.
• Communicate effectively with colleagues, service users and external professionals.
• Maintain accurate records using digital systems.

Required profile

• Experienced Care Team Leaders or Senior Support Workers with an NVQ/QCF in Health and Social Care (or equivalent).
• Ambitious Support Workers or Care Assistants with a strong track record and a desire to progress.

Required skills

• Digital record‑keeping.
