Community Nurse – Band 5 (Adult Community Nursing)

About the role

We are looking for enthusiastic Band 5 Community Staff Nurses with post‑registration experience to join our adult community nursing teams across Radcliffe, Bury, Ramsbottom and Prestwich. You will work within an Integrated Neighbourhood Team, delivering high‑quality care to patients in their homes and supporting them to stay independent.

Key responsibilities

• Provide comprehensive community nursing assessments and care planning for a diverse caseload.
• Collaborate with health and social care professionals to prevent unnecessary hospital admissions and facilitate timely discharges.
• Deliver health education and self‑management support to empower patients and their families.
• Participate in multidisciplinary team meetings and contribute to service improvement initiatives.
• Work flexible shifts between 08:00 and 20:30 across a seven‑day roster.

Required profile

• Registered Nurse with post‑registration experience (Band 5 level).
• Excellent communication skills and ability to build therapeutic relationships.
• Commitment to delivering patient‑centred care within a community setting.
• Willingness to work flexible hours and adapt to the needs of the team.
