Community Nurse – Brynmawr

About the role

The Community Nurse will join the Brynmawr District Nurse Team, delivering holistic, patient‑centred care in homes, residential settings and community facilities. This role supports early discharge, prevents unnecessary hospital admissions and contributes to the health board’s clinical futures model.

Key responsibilities

• Provide comprehensive nursing assessments and interventions in patients' homes and community settings.
• Coordinate care plans with multidisciplinary teams to ensure seamless service delivery.
• Support early discharge processes and work to reduce avoidable hospital admissions.
• Deliver health education, wound care, medication management and chronic disease monitoring.
• Maintain accurate clinical records and adhere to NHS governance and safety standards.

Required profile

• Registered Nurse (NMC) with experience in community or district nursing.
• Commitment to delivering high‑quality, person‑focused care.
• Ability to work independently and as part of a multidisciplinary team.
• Willingness to adapt to flexible working patterns as required.
