Dementia Link Worker – Post Diagnostic Support (Community Mental Health)

About the role

This permanent position is within the Post Diagnostic Support Team of the Older Peoples Community Mental Health Team (OPCMHT) in East Dunbartonshire. The role provides a minimum of one year of post‑diagnostic support to people newly diagnosed with dementia, following the Scottish Government’s 5‑pillar model.

Key responsibilities

• Act as the named contact for the person with dementia and their carer/family.
• Deliver information, advice and emotional & practical support using the 5‑pillar model.
• Help individuals and families develop personalised care plans and future care preferences.
• Monitor progress, adapt support to changing needs and ensure outcomes are achieved.
• Promote health‑promotion approaches and raise awareness of dementia.
• Collaborate with multidisciplinary professionals delivering dementia services.
• Facilitate short‑term support or educational groups for carers and families where appropriate.

Required profile

• SVQ/HNC in health or social care, or equivalent experience.
• Experience working with older adults in mental health settings.
• Knowledge of the HEAT Target for post‑diagnostic services and the 5‑pillar model.
• Understanding of mental health legislation relevant to dementia care.
