Family Services Floating Support Case Worker – Fixed Term

About the role

Family Floating Support Service is a community‑based outreach programme that works with families who have secured a home after leaving residential family services. The role supports service users by sign‑posting them to services that improve housing, health and wellbeing.

Key responsibilities

• Develop and deliver personalised support plans, assess safety and wellbeing needs, and reduce identified risks.
• Provide outreach work, key working with service users and external agencies, and advocate for best outcomes.
• Ensure health and safety standards are met and complete required wellbeing checks.
• Maintain accurate client records, manage financial transactions, and produce reports while adhering to IT security policies.
• Participate in team handovers, meetings, training, and mentor volunteers as needed.

Required profile

• Minimum Level 3 qualification in a social care field (e.g., A‑Levels, diploma in health and social care) or equivalent.
• At least 2 years paid relevant experience (or 3 years with Level 2 qualifications such as GCSEs in Maths and English).
• Good working knowledge of risk assessment, IT systems for client information, and strong written and verbal communication.
• Understanding of homelessness issues, statutory and voluntary sector resources, and health‑and‑safety standards.
