Health Care Support Worker – AHP Long Term Conditions Service

About the role

The Long‑Term Conditions service is an AHP‑led, digital‑first self‑management programme supporting patients with chronic fatigue‑related conditions such as ME/CFS, fibromyalgia, post‑viral fatigue and Long Covid. The role combines virtual group work, specialised workshops and one‑to‑one interventions delivered from home, community settings and remote bases.

Key responsibilities

• Provide direct patient care and rehabilitation, delivering delegated self‑management treatment virtually, at home or in agreed community settings.
• Adjust treatment plans within agreed parameters and report any unexpected changes in patient progress.
• Support patients experiencing complex fatigue, pain, breathlessness and reduced quality of life.
• Assist in organising and facilitating the virtual 6‑week self‑management group.
• Develop Wellness Recovery Action Plans (WRAP) with patients to support discharge self‑management.
• Perform safe manual handling and use mobility/therapy equipment as required.
• Implement therapeutic activities such as memory strategies, relaxation, anxiety and fatigue management.

Required profile

• Qualified Health Care Support Worker complying with NHSGGC Mandatory Induction Standards and Code of Conduct.
• Professional registration with CSP or RCOT and adherence to HCPC Standards of Proficiency (if applicable).
• Valid driving licence and access to transport for visits across the NHSGGC geography.
• Ability to work flexibly in a hybrid model – remote, community visits and occasional on‑site attendance.

Required skills

• Manual handling competence.
• Safe use of mobility and therapy equipment.
