About the role
Our client, Lincolnshire County Council, is expanding its Leaving Care service for young people aged 16‑25. The Leaving Care Worker – Personal Adviser will join a dedicated team in Lincoln, providing information, advice, guidance and practical support to care‑experienced youth as they transition to independent adulthood.
Key responsibilities
- Conduct statutory face‑to‑face visits with young people, both locally and nationally.
- Develop and maintain personalised Pathway Plans covering accommodation, health, education, employment, finances and wellbeing.
- Use the Signs of Safety model and the Valuing Care tool to assess needs and monitor progress.
- Provide ongoing advice and practical assistance to help young people achieve independent living skills.
- Collaborate with the young person’s network of professionals, families and support agencies.
Required profile
- Experience working with care‑experienced young people aged 16‑25.
- Ability to recognise and manage risk and vulnerability.
- Strong relationship‑building skills and a commitment to empowering young people.
- Willingness to travel for visits and attend meetings as required.
Required skills
- Familiarity with the Signs of Safety framework.
- Knowledge of the Valuing Care assessment tool.
