Outreach Recovery Worker – Support Adults in Recovery

About the role

We are seeking a compassionate and resilient Outreach Recovery Worker to join our team in Sittingbourne. You will support adults aged 18+ who are dealing with drug or alcohol issues, delivering person‑centred psychosocial interventions within an integrated healthcare framework.

Key responsibilities

• Manage a diverse caseload, conducting comprehensive assessments and risk assessments.
• Develop SMART recovery care plans tailored to individual goals.
• Deliver structured one‑to‑one and group interventions focused on harm reduction and abstinence.
• Provide practical advice, education and support to promote healthier choices.
• Collaborate with housing providers, healthcare services, mental health teams, employment services, probation and community organisations.
• Maintain accurate records and ensure compliance with safeguarding, governance and quality standards.
• Work occasional evenings, weekends and travel within the local cluster as required.

Required profile

• Passionate about supporting people to achieve positive change using a strengths‑based, recovery‑focused approach.
• Excellent relationship‑building and communication skills with diverse populations.
• Calm and professional demeanor in challenging situations.
• Experience in substance misuse or related health and social care services.
• Strong understanding of recovery, harm reduction and safeguarding principles.
• Confidence in managing caseloads, assessments and care‑plan development.
