PATCH Nurse – Paediatric Hospital at Home

About the role
The PATCH Nurse will join the innovative Paediatric Hospital at Home service at West Middlesex University Hospital. Working within the PATCH team, you will provide high‑quality assessment and treatment for acutely unwell children in the comfort of their own homes, supported by the Virtual Wards Programme and the wider NHS network.
Key responsibilities
- Independently assess and manage acutely unwell children referred to the PATCH service.
- Deliver face‑to‑face, virtual and telephone consultations, escalating care when required.
- Coordinate with hospital teams to ensure timely and effective treatment.
- Maintain accurate clinical records and adhere to NHS safety and quality standards.
- Travel to patients’ homes across the service area, requiring a valid driving licence.
Required profile
- Registered Nurse with current NMC registration.
- Experience in paediatric acute care or community nursing.
- Ability to work autonomously and make clinical decisions.
- Strong communication skills for virtual and in‑person interactions.
- Commitment to the NHS values of compassion, safety and excellence.
Required skills
- Valid UK driving licence.
