About the role
An exciting opportunity has arisen to recruit two Primary Care Health Care Support Workers (HCSW) within the Community Treatment Care (CTAC) service. The role is based in General Practices across the East Health and Social Care Partnership and works as part of an integrated nursing team and wider multidisciplinary team.
Key responsibilities
- Support the delivery of quality person‑centred nursing care and defined nursing interventions in a community treatment and care room setting.
- Collaborate with the integrated nursing team and the broader General Practice multidisciplinary team to meet the needs of the practice population and local community.
- Work safely, caring and respectfully with patients, colleagues and partners in line with NHS values.
Required profile
- Full UK driving licence and access to a vehicle (essential).
- Eligibility to work in the United Kingdom.
- Desire to develop a career in primary or community care; community/general practice experience is desirable but not essential.
