Advanced Clinical Practitioner – Care Coordination & OOH

About the role

Health Hero is seeking an independent Prescribing Clinician to join its Care Coordination and Integrated Urgent Care services across Bath, Swindon and Wiltshire. The role combines clinical assessment with system navigation to ensure patients receive the right care at the right time, reducing unnecessary hospital admissions.

Key responsibilities

• Act as a senior clinical decision‑maker, triaging and redirecting patients across ambulance, primary, community and secondary services.
• Provide real‑time clinical support to ambulance crews, identifying alternative pathways to avoid unnecessary Emergency Department visits.
• Deliver clinical assessment, diagnosis and prescribing via remote consultations, face‑to‑face visits and home visits.
• Cover out‑of‑hours urgent care shifts, including triage of NHS111 referrals and mobile/home visiting duties.
• Support the development of integrated, whole‑system care pathways and continuous improvement of coordination processes.

Required profile

• Qualified prescriber with experience in acute and urgent care settings.
• Ability to work autonomously and make senior clinical decisions.
• Strong collaborative skills to work with ambulance services, primary care, community and secondary care teams.
• Flexibility to cover both regular office‑based shifts and out‑of‑hours mobile assignments.
