Advanced Nurse Practitioner

About the role

IC24 is seeking an Advanced Nurse Practitioner to join its Hastings Primary Care Hub. The role combines clinical leadership, autonomous practice and a focus on improving outcomes for vulnerable patients, including those experiencing homelessness, mental ill‑health, substance misuse and learning disabilities.

Key responsibilities

• Provide assessment, diagnosis and treatment at the point of first contact for unregistered and vulnerable patients.
• Prescribe medications as an Independent Prescriber, ensuring evidence‑based treatment plans.
• Work within a multidisciplinary team to navigate patients to the most appropriate care pathway.
• Collaborate with local partners such as the Seaview Project to support individuals in temporary accommodation.
• Deliver safe, high‑quality care that reduces health inequalities and improves access to services.

Required profile

• Registered Advanced Nurse Practitioner with Independent Prescribing qualification.
• Experience in primary care or community health settings.
• Ability to work with unregistered patients and those with complex physical, psychological and social needs.
• Strong clinical decision‑making skills and commitment to patient‑centred care.
