Assessment Social Worker – Social & Emotional (SE)

About the role

We are looking for a compassionate Assessment Social Worker specialising in Social and Emotional (SE) assessment to join our inclusive team in Reigate. The role focuses on delivering thorough assessments, developing personalised care plans and supporting individuals to modify unauthorised behaviours.

Key responsibilities

• Conduct comprehensive social and emotional assessments to identify client needs, strengths and challenges.
• Design and implement individualised care plans that address unauthorised behaviours and promote wellbeing.
• Collaborate with multidisciplinary teams—including healthcare providers, educators and families—to ensure coordinated support.
• Maintain accurate, confidential case records in line with organisational policies and legal requirements.
• Advocate for clients’ rights and facilitate access to necessary services and resources.
• Lead group sessions and workshops focused on social and emotional development.
• Monitor client progress, evaluate outcomes and adjust plans as needed.
• Participate in case conferences, team meetings and ongoing professional development.

Required profile

• Degree in Social Work (BSW, MSW) or equivalent qualification.
• Registration or eligibility for registration with a recognised professional social work body.
• Experience conducting social and emotional assessments, particularly related to unauthorised behaviour intervention.
• Strong interpersonal and communication skills with cultural competence.
• Ability to work independently and as part of a multidisciplinary team.
• Commitment to diversity, equity and inclusion principles.
• Knowledge of relevant legislation, policies and safeguarding best practices.
• Excellent organisational skills and effective caseload management.
