Band 6 Dual Diagnosis Nurse – Part‑time

About the role

We are seeking a compassionate Band 6 Dual Diagnosis Nurse to join The Forward Trust in West Thurrock on a part‑time, permanent basis. The role involves supporting individuals who experience both substance misuse and mental health challenges, delivering person‑centred care across community and inpatient settings.

Key responsibilities

• Conduct comprehensive clinical assessments for clients with co‑occurring substance misuse and mental health conditions.
• Develop, implement and regularly review personalised care and risk‑management plans.
• Provide evidence‑based interventions such as motivational interviewing, relapse prevention, harm reduction and trauma‑informed approaches.
• Support clients through community and inpatient detox pathways and promote overdose prevention, including naloxone provision.
• Deliver one‑to‑one and group therapeutic sessions and monitor changes in mental health, escalating concerns when required.
• Collaborate with Community Mental Health Teams, GPs, housing providers, social care, criminal justice agencies and voluntary organisations.
• Contribute to service development, audits, quality‑improvement projects and staff training.

Required profile

• Registered Nurse (NMC‑registered) with experience in substance misuse, mental health, or dual‑diagnosis settings.
• Demonstrated ability to work autonomously and as part of a multidisciplinary team.
• Strong communication skills and a commitment to recovery‑focused, person‑centred care.
