Band 6 Team Sister / Therapist – Community Transfer of Care Hub

About the role

As a Band 6 Team Sister / Therapist within the Community Transfer of Care Hub at Gravesham Community Hospital, you will lead the safe discharge of patients from acute hospitals to appropriate community settings, assessment beds or their own homes. The role focuses on complex discharge planning, coordinating with acute, social care and community partners to ensure timely, patient‑centred transitions.

Key responsibilities

• Provide operational, clinical and professional leadership to the Hub team.
• Manage referrals into Discharge to Assess pathways across Dartford, Gravesham, Swanley and Medway.
• Coordinate with acute hospitals, social care teams and care providers to improve patient flow.
• Support patients and families to understand and access the right care at the right time.
• Contribute to service development and continuous improvement initiatives.

Required profile

• Registered Nurse or qualified Therapist with current registration.
• Experience in complex discharge planning and multi‑agency collaboration.
• Strong leadership and communication skills.
• Commitment to the NHS “Home First” approach.
