Care Coordinator

About the role

We are looking for a highly organised Care Coordinator to join our team in Southend‑on‑Sea. The role involves managing care coordination for a defined client patch, working closely with field staff and local authorities to ensure high‑quality, compliant care delivery.

Key responsibilities

• Manage and oversee daily rotas, ensuring efficient scheduling and continuity.
• Own a defined client patch, monitoring visits and maintaining accurate documentation.
• Guide and mentor coordinators and administrative staff, providing support and feedback.
• Maintain meticulous records and ensure compliance with CQC standards.
• Process local‑authority packages and collaborate closely with field staff.
• Handle escalations, concerns and participate in the on‑call rota.
• Identify and propose improvements to coordination workflows.

Required profile

• Level 3 Health & Social Care qualification.
• Minimum 2 years’ experience as a Care Coordinator in domiciliary care.
• Leadership or mentoring experience within a coordination setting.
• Strong knowledge of Access Care Planner or a similar ECM system.
• Excellent written communication and record‑keeping skills.
• Proficiency with Excel and data management.
• Full UK driving licence and access to a car.
