Clinical Pharmacist – Maternity Cover (PCN)

About the role

The Confederation, Hillingdon CIC is seeking a Clinical Pharmacist to provide maternity cover within its Primary Care Network (PCN). Working as part of a multidisciplinary team, you will help optimise medication use, support safe prescribing and deliver patient‑focused pharmacy services across GP practices, care homes and community settings.

Key responsibilities

• Conduct clinical medication reviews for patients in practice, care homes and domiciliary settings.
• Develop and run repeat prescription reauthorisation processes and medicines transfer‑of‑care pathways.
• Identify and manage polypharmacy, offering de‑prescribing recommendations and optimisation strategies.
• Provide face‑to‑face, telephone and home‑visit clinics for conditions such as respiratory, cardiovascular and diabetes.
• Collaborate with GPs, nurses and senior pharmacists to improve prescribing safety and adherence.
• Participate in quality‑improvement projects and support care home staff in medication safety.

Required profile

• Registered pharmacist with the General Pharmaceutical Council (GPhC).
• Experience in primary‑care or community pharmacy settings is desirable.
• Willingness to complete the 18‑month CPPE Primary Care Pharmacy Education Pathway or hold a Health Education England exemption.
• Ability to work autonomously and within a multidisciplinary team.
