Community Mental Health Social Worker (Neath Port Talbot)

About the role

Join the Community Mental Health Team at Neath Port Talbot County Borough Council, based in the Civic Centre, Neath. You will work with adults across the locality, managing a caseload of complex mental health needs and delivering a range of statutory assessments.

Key responsibilities

• Manage a caseload of adults with complex mental health and social care needs.
• Conduct capacity, Best Interest, CHC and financial assessments within the Council's charging framework.
• Develop outcome plans in partnership with individuals, families and carers.
• Collaborate with multi‑disciplinary professionals and external agencies.
• Attend and contribute to case conferences, reviews and safeguarding meetings.
• Take appropriate protective action for people at risk, including AMHP duties where relevant.
• Work a hybrid pattern: four days office‑based and one day remote, subject to staffing levels.

Required profile

• Relevant Social Work qualification.
• Registration with Social Care Wales or Social Work England.
• Enhanced DBS on the update service (or willingness to obtain one).
• Three years of professional referencing.
• Full, clean driving licence and access to a car.
• Commitment to work in the Neath Port Talbot area.

Required skills

• Expertise in conducting capacity, Best Interest, CHC and financial assessments.
• Strong case‑management and multi‑agency collaboration abilities.
• Knowledge of safeguarding procedures and AMHP responsibilities.
• Ability to develop person‑centred outcome plans.
