Community Nurse – Band 5 (Aberystwyth)

About the role

Hywel Dda University Health Board is seeking a Band 5 Community Nurse to join its Community Nursing Team in Aberystwyth. The role involves delivering high‑quality, complex care to patients in their own homes, supported by an experienced District Nurse Team Leader.

Key responsibilities

• Manage a delegated caseload within the Community Nursing Team.
• Provide comprehensive care management to patients at home or in clinic settings.
• Maintain high standards of care for patients, families, and carers.
• Liaise with staff, patients, carers, and external agencies to ensure appropriate delegation and risk management.
• Communicate effectively with all members of the care package.
• Travel between multiple sites promptly to meet service demands.
• Work a 7‑day rota, covering weekends and bank holidays from 8 am to 8 pm.

Required profile

• Current employee of Hywel Dda University Health Board.
• Qualified Band 5 nurse with experience in community settings.
• Willingness to work a flexible, 7‑day schedule.
• Ability to travel between sites in a timely manner.
• Welsh language ability is desirable but not mandatory.
