About the role
This 9‑month secondment position joins the Home First Discharge Team in the West, supporting effective discharge planning across Gwynedd and Monmouthshire. The role is based primarily at Ysbyty Gwynedd with occasional community rotations, offering a unique opportunity to influence patient flow from hospital to home.
Key responsibilities
- Provide specialist knowledge and leadership to acute and community staff on discharge planning.
- Implement the Health Board’s Discharge to Recover & Assess (D2RA) strategy.
- Collaborate with local authority and voluntary partners to develop models of care that enable timely hospital discharge.
- Identify barriers to discharge, lead work‑streams to address them, and monitor outcomes.
- Deliver education and training programmes for multidisciplinary teams.
- Prepare and maintain accurate documentation and assessments in line with statutory requirements.
Required profile
- Registered Nurse with current registration and experience in acute or community settings.
- Demonstrated interest or experience in discharge processes and care coordination.
- Ability to work collaboratively across health, social care and voluntary sectors.
- Excellent communication and presentation skills.
- Willingness to obtain line‑manager permission for internal secondment.
Required skills
- Knowledge of the Discharge Process and Funded Nursing Care.
- Understanding of Continuing Healthcare & Social Well‑Being (Wales) Act 2014.
- Proficiency in clinical assessment and documentation.
