General Practice Nurse – Community Care

About the role

The General Practice Nurse will join the Chapel Street Enhanced Care Home Team at Brierley Hill Health and Social Care Centre. Working under the Trust’s management, you will deliver nursing care to residents in both residential and nursing settings, adhering to the NMC Code of Conduct.

Key responsibilities

• Assist and perform routine patient‑care tasks as directed by the Nurse Lead and GPs.
• Carry out cytology sampling, venepuncture and adult vaccinations, including COVID‑19.
• Request pathology tests such as urine cultures and swabs.
• Manage long‑term conditions (diabetes, hypertension, COPD, asthma, CHD, CVA) and maintain QOF standards.
• Order and stock vaccinations, ensuring appropriate inventory levels.
• Follow agreed clinical protocols and refer to GPs when necessary.
• Participate in infection‑control audits and maintain accurate patient records.
• Use practice administration systems (EMIS, CMCS, ICE, Docman) for documentation and communication.

Required profile

• Registered with the Nursing and Midwifery Council (NMC) and eligible to practice in the UK.
• Ability to work independently and as part of a multidisciplinary team.
• Resilience under pressure and strong organisational skills.
• Commitment to continuous professional development and patient safety.

Required skills

• Proficiency with EMIS Web (training provided).
• Familiarity with CMCS, ICE and Docman administration platforms.
• Competence in venepuncture, cytology sampling and adult vaccination delivery.
