Healthcare Support Worker – Home First Service

About the role

The Home First Service is seeking an experienced Healthcare Support Worker to provide person‑centred care in homes across the Berwickshire and Cheviot localities. You will join a multidisciplinary team delivering integrated community health services alongside Hospital at Home, District Nursing and other professionals.

Key responsibilities

• Deliver high‑quality, person‑centred care to patients in their own homes.
• Work alongside registered nursing staff and coordinate care with other health professionals, patients and families.
• Support service delivery across additional localities (Central, Teviot & Tweeddale) when required.
• Make confident decisions without direct supervision and adhere to integrated care pathways.
• Maintain flexible working patterns, covering weekdays and weekends on a rotational basis.

Required profile

• Motivated, adaptable and able to work independently.
• Full UK driving licence with business insurance.
• Willingness to work 08:00‑20:30 across a 7‑day roster.
• Eligibility to work in the United Kingdom.
• Prepared to undergo criminal records checks and PVG scheme registration if required.
