Home First Nurse Team Lead

About the role

The Home First Service within NHS Borders is seeking an experienced Registered Nurse to lead a multidisciplinary team delivering short‑term reablement and rehabilitation in patients' homes. This Band 7, permanent position offers the chance to shape community‑based care across several localities while working closely with acute, district nursing, allied health, and social care partners.

Key responsibilities

• Provide leadership and supervision to Home First Nurse Coordinators and Health Care Support Workers.
• Develop, implement and evaluate reablement and nursing care programmes for patients at home.
• Manage day‑to‑day service delivery across Berwickshire, Central, Cheviot, Teviot & Tweeddale.
• Make confident clinical decisions without direct supervision.
• Line‑manage both registered and unregistered staff.
• Support quality improvement and service development initiatives.
• Work flexibly, including rotating weekdays and weekends, to meet service needs.

Required profile

• Registered Nurse qualification with current registration.
• Experience in community nursing, reablement or rehabilitation.
• Strong leadership and decision‑making abilities.
• Full UK driving licence and willingness to travel across multiple localities.
• Ability to work independently and manage a diverse team.
