Long Term Care Specialist Nurse

About the role

The Long Term Care Specialist Nurse will work within the NHS Long Term Care Service, providing autonomous assessment and support for patients transitioning from acute care to nursing home placement. The role bridges secondary care, community services, and care homes, ensuring person‑centred, seamless care.

Key responsibilities

• Assess and monitor individuals in nursing homes, private residences, and community settings.
• Support care home staff and community teams with training and guidance.
• Facilitate Continuing NHS Healthcare (CHC) and NHS Funded Nursing Care (FNC) processes.
• Assist with Direct Payments for Health to promote patient choice.
• Collaborate with multidisciplinary teams to implement the Long Term Care Pathway.
• Travel across the Hywel Dda area and neighbouring counties as required.

Required profile

• Registered Nurse with an Acute & Community or Mental Health background.
• Strong communication skills and ability to work both independently and as part of a team.
• Willingness to travel throughout the health board region.
• Desirable: ability to speak Welsh.
