Nurse or Paramedic – NHS@Home Community Care

About the role

Join the NHS@Home service to deliver acute, specialist care to patients in their own homes across Bristol, North Somerset and South Gloucestershire. You will work alongside a multidisciplinary team, using both face‑to‑face visits and digital technology to monitor and treat patients safely at home.

Key responsibilities

• Provide clinical assessments and virtual monitoring for patients receiving NHS@Home care.
• Deliver acute treatments at home, including venepuncture, ECG, bladder scanning and wound care.
• Develop, implement and evaluate personalised care plans with the multidisciplinary team.
• Observe and record vital signs, recognise deterioration and act according to local protocols.
• Communicate effectively with patients, carers and health‑care professionals to ensure safe, compassionate care.

Required profile

• Qualified Nurse or Paramedic registered with the appropriate UK regulator.
• Strong clinical judgement and ability to work independently in community settings.
• Excellent communication skills and a patient‑centred approach.
• Willingness to travel across the service area and adapt to both face‑to‑face and virtual interactions.
