Rehabilitation Community Nurse – Band 6 (Fixed‑term, Part‑time)

About the role

The North West Rehabilitation Service is seeking a Band 6 Registered Nurse to join a multidisciplinary team delivering community‑based rehabilitation for older and disabled adults with complex needs. This fixed‑term, part‑time position focuses on assessment, treatment, and safe discharge planning, promoting independence and self‑management.

Key responsibilities

• Manage a complex clinical caseload independently, applying specialist nursing knowledge.
• Deliver person‑centred rehabilitation care aimed at maximising independence.
• Collaborate with physiotherapists, occupational therapists, and other professionals to support assessment, treatment, and discharge planning.
• Supervise support workers and nursing students.
• Provide professional nursing leadership, guidance, and development within the team.

Required profile

• Registered with the Royal College of Nursing (RCN).
• Experience in community nursing and multidisciplinary teamwork.
• Relevant background in neurology.
