Remote Pharmacist Prescriber – Fully Remote

About the role

This fully remote position supports an innovative Hospital‑at‑Home service by delivering telephone‑based medication reviews. The role focuses on optimising patients' medication regimens, identifying safety issues early, and making prescribing decisions where appropriate.

Key responsibilities

• Conduct structured medication reviews with patients over the phone.
• Assess current medications for safety, effectiveness, and adherence.
• Discuss symptoms, side effects, and treatment outcomes with patients.
• Amend, initiate, or discontinue medications in line with clinical guidelines.
• Manage routine medication‑related alerts and clear them promptly.
• Identify red‑flag situations and escalate to nurses or clinicians.
• Document all consultations and medication changes accurately in clinical systems.
• Provide clear advice, reassurance, and education to patients.

Required profile

• Registered Pharmacist with Independent Prescribing qualification.
• Experience conducting medication reviews in primary care, community, or remote settings.
• Strong clinical decision‑making and risk‑assessment abilities.
• Confident telephone consultation style and ability to work autonomously.
• Excellent documentation, organisational, and communication skills.
