Specialist Clinical Pharmacist – Community Services

About the role

The Specialist Clinical Pharmacist will join the Connecting Care Pharmacy team to provide medicines optimisation for patients discharged from hospital. Working mainly in patients' homes, you will conduct clinical medicine reviews, educate patients on medication use, and monitor outcomes as part of a multidisciplinary team.

Key responsibilities

• Review patients’ medication regimens and carry out clinical interventions where needed.
• Provide medication education and assess adherence during home visits.
• Collaborate with medical and nursing staff to develop and maintain pharmacy policies and guidelines.
• Identify and report risk management issues or clinical incidents.
• Generate and analyse reports on medication usage, expenditure and safety.
• Participate in audits and support formulary and drug usage initiatives.

Required profile

• Registered pharmacist with the General Pharmaceutical Council.
• Post‑graduate diploma in clinical pharmacy or equivalent experience.
• Previous experience working in a clinical environment.
• Ability to work effectively within a multidisciplinary team.
• Willingness to undertake Non‑Medical Prescribing training if not already qualified.
