Specialist Pharmacist – Hospital at Home

About the role

The NHS Borders Pharmacy Department is seeking a Specialist Pharmacist to join its Hospital at Home (H@H) team. You will deliver high‑quality, safe, person‑centred clinical pharmacy services to patients receiving hospital‑level care in their own homes.

Key responsibilities

• Provide specialist pharmaceutical advice and medicines optimisation for H@H patients.
• Conduct pharmacist‑led medication reviews and patient consultations in person, by telephone or video, including polypharmacy reviews.
• Practice as an independent prescriber (or undertake prescribing training) to manage a defined caseload.
• Collaborate with the multidisciplinary team to improve medicines safety, support safe transfer of care and reduce avoidable admissions.
• Contribute to service development, policies, pathways and governance activities such as audit, incident learning and antimicrobial stewardship.
• Support education, training and supervision of colleagues and learners.

Required profile

• Registered pharmacist with specialist experience.
• Willingness or ability to act as an independent prescriber.
• Experience in medication reviews, polypharmacy management and patient‑centred consultations.
• Ability to work collaboratively within a multidisciplinary team.
