Young Person Support Worker – Bespoke Support Role

About the role

The Young Person Support Worker provides trauma‑informed, day‑to‑day care for children and young people as outlined in their care plans. Working within a family‑like setting, you will promote safety, well‑being and personal development while supporting education, employment and community engagement.

Key responsibilities

• Deliver daily support in line with the child’s care plan, including meal preparation, household cleaning and personal hygiene.
• Encourage healthy lifestyle choices through structured routines and family‑time activities.
• Assist with education, training, employment pathways and facilitate attendance at health appointments.
• Promote positive relationships with neighbours, community members and broader support services.
• Arrange and accompany the young person to cultural, leisure and community activities.
• Provide transport when required, holding appropriate business use insurance.
• Maintain a daily logbook and incident records, sharing securely during handovers.
• Report any incidents of self‑harm, substance misuse, missing persons or other risks immediately through the emergency duty team.
• Follow safeguarding, health and safety, data protection and the Philomena missing‑person protocol.
• Support transition plans and move‑on arrangements.

Required profile

• Understanding of trauma‑informed care and child protection principles.
• Ability to work independently while adhering to organisational policies.
• Strong communication skills for liaising with social workers, families and external agencies.
• Reliability in maintaining accurate records and responding to emergencies.
• Valid driver’s licence and appropriate business use insurance.
