Diabetes Specialist Nurse – Possilpark Health & Care Centre

About the role

The Diabetes Specialist Nurse will join the Possilpark Health & Care Centre as an autonomous practitioner delivering expert diabetes care to patients and their families. This permanent, full‑time role requires a current NMC UK PIN and offers the opportunity to lead and develop multidisciplinary diabetes services within NHS Greater Glasgow and Clyde.

Key responsibilities

• Assess, plan, implement and evaluate individual care programmes for patients with diabetes.
• Develop and deliver comprehensive care packages, including lifestyle advice and personalised glycaemic targets.
• Provide education and support to patients, carers and multidisciplinary team members.
• Initiate and manage insulin therapy and other diabetes treatments, including sick‑day rules.
• Coordinate seamless care from admission to discharge, ensuring effective communication across the MDT.
• Supervise and mentor nursing staff, adhering to professional guidelines and protocols.

Required profile

• Registered Nurse with a current NMC UK PIN.
• Significant post‑registration experience in diabetes care and relevant education.
• Ability to work autonomously and lead nursing staff.
• Valid driver’s licence; willingness to drive as part of the role (PVG scheme required).

Required skills

• Diabetes management and insulin therapy.
• Patient and carer education on glycaemic control.
• Assessment of suitability for therapeutic treatments.
• Management of diabetic complications and sick‑day rules.
